Medifast is a protein sparing diet that concentrates on restricting calorie and carb intake. The restriction of the carb intake is so that individuals enter into a mild ketosis state which makes the body burn ketones for energy and hence burns fat not protein.
You are also supposed to restrict your exercise. None for the first three weeks and then gradually build up moderate exercise. If you are concerned about muscle loss then I would recommend conditioning exercises such as strength training, pilates, some forms of yoga, etc, but medifast is a diet that promotes fat loss not muscle/protein loss when done correctly.
